Impact of Home Pharmacy Care on Adherence of Chronic Renal Failure Patients Undergoing Hemodialysis in a Hospital in Indonesia

Abstract

One of the goals of home pharmacy care in patients with chronic renal failure is to improve adherence to taking medication.
Renal failure is a chronic disease that requires long-term therapy. This research aimed to analyze the effectiveness of home
pharmacy care on the adherence to taking medicine in chronic renal failure patients undergoing hemodialysis in a hospital
in Indonesia. This research conducted using a quasi-experiment method with a one-group pretest-posttest design. Data took
by consecutive sampling. The inclusion criteria were patients diagnosed with chronic renal failure who are undergoing
hemodialysis with age over 18 years and could communicate well. Patients provided with a standard validated modified
Medication Adherence Rating Scale questionnaire to assessing medication adherence before and after the intervention. This
research conducted pharmacy service at home by giving one-time counseling, evaluation of drug use, and storage of drug
for the patient of chronic renal failure undergoing hemodialysis. Data were analyzed using Paired sample T-Test and
Wilcoxon test. The research respondents were 45 people. Statistical analysis of pretest and posttest data indicated that home
pharmacy care improved patient adherence from average ± SD 6.62 ± 1.98 to 8.38 ± 1.58. Statistically, the significant
association exists between medication adherence before and after the intervention (p <0.05). Counseling by pharmacists
with booklet impact on patient's adherence to chronic renal failure undergoing hemodialysis.
