Plasma Fibrin/Fibrinogen as Tissucol Adhesive Material in Periodontal Surgery: Part 1, Hemostasis and Flap Adhesion

The present investigation was undertaken to study the efficacy of fibrin adhesive system [tissucol] as a substitution
material to the traditional silk suturing and to compare the clinical findings obtained from both, suture Vs tissucol. Thus,
two groups[tissucol group TG, and Suture group SG] were taken, each of 21 adult periodontitis cases of nearly equal age.
Those, were subjected to a repositioned flap surgery, apically repositioned flap surgery and modified Widman flap surgery
procedures. Hemostasis time [HT], Flap adaptation time [FAT], Postoperative discomfort time, pain and swelling [PDT]
and clinical healing achievement time [CHAT] of TG were compared to that of SG. Result showed that fibrin adhesive
system provides a fast hemostasis action and excellent flap fixation and adaptation. The introduction of tissucol shortened
significantly the clinical healing period of wounded area[CHAT] and as well the postoperative discomfort time [PDT]. To
discuss the clinical results, it appeared, the intragroup comparison revealed there were a significant gain in application
time, shortening procedure time, absence of retard healing in stiches needle punctures, smooth, homogenous gingival tissue
adaptation, absence of postoperative discomfort, faster blood clot formation hence a significant gain in hemostasis and
bleeding time
